Post-Masters Certificate Forms

All Post-Master's Certificate students will need to fill out the following graduation/certificate clearance forms as they draw near their graduation month:

• Application for Certificate
  Please complete and return this form to OSCAS no later than the first day of the month you plan to graduate.
   
• Certificate Plan
  Please complete this form, and have your specialty coordinator sign it, before sending to OSCAS for processing. Form should be sent in as soon as all courses are complete and final grades have been earned.
   
• Commencement Attendance Form
  Please complete and return to OSCAS no later than the first day of April, even if you don’t plan to attend the ceremony.
