Nomination of Sages

The Planning Committee for the 2009 Summit of Sages invites you to nominate groups that have been instrumental in fostering innovation and inspiration through the creation of new models of care delivery.
BACKGROUND
In 2004 and 2007, the Densford International Center for Nursing Leadership at the University of Minnesota School of Nursing hosted international conferences highlighting the work of Sages (extraordinary leaders). Approximately 400 passionate individuals attended each of these conferences and thoughtfully examined the issues of leadership and social justice.
The upcoming 2009 Summit of Sages will build on the work from these two conferences and recognize Innovation, Inspiration and Transformation in health care delivery. Because individuals can not create revolutions by themselves, the focus of this Summit will be on clusters of people - teams, groups, partnerships - that together have created new ways of delivering health care. As is often the case, one inspirational leader may have spearheaded the innovation - but to fully realize it required the strength of many.
SUBMIT NAMES !!
We invite you to submit the name of a nursing leader and his/her team who have developed a revolutionary care model or practice that has truly shaped nursing and health care over time, and provided an exciting way to improve care delivery to patients or populations. Examples include the development of new models of care delivery, the creation of new health care environments, the establishment of new roles for patients and their families - any innovation that has truly transformed the health care that individuals and/or groups receive. The deadline for nominations is August 6, 2008.
Nomination
Please describe (in 300 words or less) the innovation that was developed, and the ways in which it transformed our thinking and approach to care of a particular population. Give an example of the broader impact that this innovation has achieved.
Individuals may nominate one or more pairs or groups; self-nominations are welcome.
