Sages These six extraordinary women launched bold new models of care with partners and in groups, transforming healthcare delivery:  | Jeannine Bayard, MPH, APRN More than 20 years ago, Jeannine Bayard and her colleague Ruth Ann Jacobsen developed a revolutionary care model that served as the foundation for the company they founded—Evercare, which has since become one of the nation's largest care coordination programs. |  | Jody Chrastek, MSN, CHPN As coordinator of Children’s Pain and Palliative Care Services, Jody Chrastek and her colleagues established a one-of-a-kind program to help young patients and their families receive comfort and care. The program also helps health care providers in communities receive training and support to extend this work. |  | Loretta Ford, EdD, RN, PNP, FAAN, FAANP In 1964, Loretta Ford, in collaboration with Dr. Henry K. Silver, designed an educational program for public health nurses focusing on well-child care, normal growth and development, family education and empowerment, and preventative health measures. Thus, the nurse practitioner movement was launched. |  | Ruth O’Brien, PhD, RN, FAAN Three decades of rigorous research, under the leadership of Dr. Ruth O’Brien and her colleagues, evolved the Nurse Family Partnership into a national evidence-based, nurse-home visiting program that improves the health, well-being, and self-sufficiency of low-income, first-time parents and their children. |  | Ruth Watson Lubic, CNM, EdD, FAAN, FACNM Surmounting challenges by believing that she was doing the right thing, Ruth Watson Lubic has been a vigorous and effective advocate for nurse midwifery. Rosemary Gibson, MSc Rosemary Gibson is a senior program officer at the Robert Wood Johnson Foundation. A provocative speaker on how the health care system has failed us and must change, her work has transformed hospital-based palliative care programs through training programs for physicians at more than 800 hospitals. She is a passionate advocate for including patients and families as full partners in care planning and delivery. Gregg Vanourek, MSc, MBA Gregg Vanourek is a founding partner of New Mountain Ventures, a personal leadership development firm, and co-author of the nationally acclaimed book, Life Entrepreneurs: Ordinary People Creating Extraordinary Lives.
