Elderberry Institute: Living at Home/Block Nurse Program homepage

For over 20 years, the Living at Home/Block Nurse Program has helped elderly people live healthy and safe lives in their own homes. The community owned, nonprofit organization has been giving community elders the in-home health care and social support they need to remain as independent as possible and in their own homes. The website gives very thorough information regarding the programs offered. Each neighborhood program and contact information is available by clicking on the "Local Programs" link from the homepage given above. The Block Nurse Program seems to be a combination of nurses and people from the community that volunteer to help seniors in their neighborhoods. Each program is geared towards the needs of the specific neighborhood, so they don't all offer the same services

The Minnesota Board on Aging serves as a gateway to services for seniors and their families through the multiple agencies it supports. They help with housing options, home care options, transportation services, nutritional services, and provide information regarding issues such as health insurance and prescription drug plans. They can also provide legal assistance.

Eldercare Partners is a nonprofit organization that works closely with other nonprofit organizations in the Twin Cities area. They serve older adults and their families by providing caregiver resources and geriatric care management services. Amherst H. Wilder Foundation

This is a nonprofit organization that collaborates with Eldercare Partners. The Wilder Foundation offers adult day health programs, senior meals, household maintenance programs, a dental program and many more programs offering support to older adults.

Dakota Area Resources and Transportation for Seniors (DARTS)

DARTS is a nonprofit organization that works with Eldercare Partners. Aside from transportation services for seniors, DARTS provides homemaking services - indoors and outdoors, caregiver support groups and education programs, and respite care.

Human Services, Inc.

Human Services, Inc. is a nonprofit organization that collaborates with Eldercare Partners. They offer services such as adult day programs, community support services, housing services, transportation, Meals on Wheels, and Congregate Dining at four locations in Washington County.

Senior Community Services is also affiliated with Eldercare Partners. They offer programs under two categories: "Supporting Frail Elders and their Families" and "Supporting Positive Aging." Programs offered include the HOME program, counseling, case management, and an outreach program. Volunteers of America - Minnesota

Volunteers of America works in collaboration with Eldercare Partners. They have a wide variety of programs for people of all ages - including programs geared towards seniors. Programs for seniors include Caregiver Services, Senior Nutrition, and Senior Support Services.

Catholic Charities of St. Paul and Minneapolis Aging Services

The Catholic Charities of Saint Paul and Minneapolis Aging Services offers bcase management programs to seniors in Ramsey and Hennepin counties. Respite care services are provided in Ramsey, Hennepin, Scott, LeSueur and Carver counties.
