Module 3 Title: Providing Transition Services to Children and Youth with Special Health Begin course |  | Awarded Contact hours: ANCC 1.0; MNBON 1.2 Purpose and Description Identify and provide transition services to children and youth with special health needs. Viewing this presentation will assist you in identifying ways to assist parents and children during times of transition. It will provide awareness of ways transition planning occurs at many stages effecting children and adolescents health needs. Objectives of the Module: On completion of this assessment, the learner will: - Identify the unique contributions nursing in transition services for young persons with special health care needs.
- Demonstrate an understanding of ways to incorporate transition planning throughout the developmental stages of childhood and adolescence.
Target Audience This module is appropriate for nurses, social workers, physicians and psychologists, and public health and health care professionals who are involved in the care and treatment of children. Learning methods This web-based instructional module was created using Flash technology. Features include graphics, interactive questions, hyperlinks, video and narration. For your convenience the module has been formatted in short time frames. For successful completion and to receive your certificate, you are required to view all module components. Registration To meet our grant reporting requirements, we ask all users to complete a post-module evaluation Form. All fields are required in order to receive Continuing Education contact hours. After you complete the registration, you will be sent a certificate by email. We ask that you bookmark the course listing page if you will need to stop before completing the module. Following the successful completion of the learning module you will be asked to complete a module evaluation. Elect either the certificate for American Nurses Credentialing Center (ANCC) or Minnesota Board of Nursing (MN BON) credit. Print the certificate for your records. Note: If you need a generic certificate of completion for your professional organization, select the Minnesota Certificate. Contact hours are calculated based on module review, assessments, exercises and required readings (as appropriate). Readings are calculated via words per page with an estimated time formula for each required reading and/or website. Accreditation Statements: Each module is assigned contact hours based on the content and evaluation. American Nurses Credentialing Center University of Minnesota School of Nursing, Office of Practice, Business and Professional Development is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation. This program is awarded 1.9 contact hours. Minnesota Board of Nursing This program is designed to meet the Minnesota Board of Nursing requirements for continuing education. It is the responsibility of the individual nurse to determine if the course fulfills that requirement. This program expires 2 years from date completed. It is the responsibility of the individual nurse to determine if the course fulfills your State Board of Nursing requirement. Please check with your Board to insure appropriate credit. This program is awarded 2.3 contact hours. Other Healthcare Providers It is the responsibility of each participant to determine if the program meets the criteria for licensure or recertification for their discipline. Each participant will receive a certificate of attendance. Please check with your accrediting organization for complete requirements. Author Bio-sketch and Credentials Barbara Peterson, RN CNP, has had over 30 years of experience in providing care to children with special health care needs and their families. Currently, Barbara works at Children's Hospitals and Clinics of Minneapolis as a Pediatric Nurse Practitioner in Psychiatry. She has a particular interest in the developmental transitions that occur for children and their families. She has participated in educational programs specific to the topic of transition both as a participant and also as a presenter. In addition she has personal experience in parenting an adult son with Down syndrome. Celia (Ceci) Shapland, RN MSN, has worked in the area of disability advocacy for over 20 years. She has been awarded numerous government grants to develop programs for youth with special health care needs planning transition in health care. Ceci has developed materials, written articles and published information for youth with special health care needs and their families. Currently, Ceci is a consultant for the Healthy and Ready to Work National Resource Center and the Minnesota Department of Health, Children with Special Health Needs Program.
