100 Distinguished Alumni

The School of Nursing has nearly 10,000 alumni; many have made great achievements to advance health care or are doing significant work in the nursing profession. In celebration of the School of Nursing Centennial in 2009, the Nursing Alumni Society Board will be honoring 100 Distinguished Nursing Alumni. Your nominations are requested (you may self-nominate) for living alumni who have profoundly impacted families, communities, the college or the nursing profession at the local, state, national or international level. Completing the Nomination Form

You have two options for completing the forms:

• You may print the forms and complete them with a typewriter or an ink pen.
• You may complete the forms from within your web browser and print them.

If you choose to complete the forms from within your web browser:

• You will need the Adobe Acrobat Reader to access all of the forms.
• You may use either the Tab key or the mouse to move from field to field.
• You must complete and print an entire document before logging out of Adobe Acrobat. Depending on the type of browser you use, you may not be able to save an incomplete form.  If you log out before the form is completed, you may lose your work.
• To print a form, be sure that the cursor has been clicked outside the text boxes. The information you've entered on the form may not print if the cursor is activated inside any text box.
